Applying for a Policy

To assist us in effecting an Industrial Disease Policy we require the following details:
  • Name of Legal entity requiring cover.
  • Postal address of the legal entity.
  • Street address of the legal entity.
  • WorkCover number.

The Industrial Disease Policy is issued to a legal entity. All subsidiary companies are individual legal entities and a separate Industrial Disease policy must be issued for each subsidiary company if they have employees who work on or visit mine sites.
