Equipment Requests

For equipment requests over $1500, an Equipment Request Form should be completed by a suitably qualified prescriber with relevant experience in prescribing the category of equipment.

For equipment requests under $1500, an email or letter to the CIS Scheme Care Services Coordinator outlining the item, cost and rationale may be sufficient.

Allied Health Assessment and Therapy

Therapy goals must be in SMART format – Specific, Measurable, Achievable, Relevant and Time-bound.
