Make a crash report and injury claim

If you have been in a crash, please click on the link that best describes you. It will take you to the crash reporting website where you can provide details about the crash and any injuries you have sustained. The information is sent to WA Police and us.

Once we have you details, we will start the claims process and send you confirmation.

Driver / Vehicle Owner

If you are the driver/owner of a vehicle involved in a crash, you must report it and you can do so via the crash reporting site. You don't need to visit a police station or report it via another c

Passenger, Pedestrian or Cyclist

If you were a passenger, pedestrian or cyclist involved in a crash, notify us of your injuries via the crash reporting site. You may be eligible to have your treatment paid to help you recover.

An injured person is required to make a claim as soon as practicable following a crash. See the Limitations Period fact sheet for more information.
