Report a crash and injuries

If you have been in a crash, please report it and let us know what injuries you sustained. You can provide details using the online form available on the crash report website.

We use the information given to determine what support we can provide to you. We also share the information with the WA Police.

An injured person is required to make a claim as soon as practicable following a crash. See the Limitations Period fact sheet for more information.
